New Patient Registration Forms

For your convenience, you can print and complete the required New Patient Registration Forms ahead of time and bring them with you to your first appointment!

Click here or on the icon below and print the entire packet of forms (11 pages). Fill out pages 1-9 and bring them with you to your first appointment with Dr. Friedberg. Pages 10 and 11 are for you to keep.

If you have any questions, our staff can answer them at the time of your initial exam. 

We ask that you arrive 30 minutes early for your first appointment as a new patient to ensure all paperwork is complete and your insurance information is up to date.  

PLEASE NOTE: If you are having trouble downloading and printing our forms, this is due to a setting in your computer. If you are unsure how to adjust your computer's settings, we kindly ask that you simply complete the forms (we will supply them to you) in our office when you arrive for your appointment.

We are unable to provide computer technical support over the phone.

In addition to these forms, please also bring your insurance card(s), photo I.D., and any co-payments. Our office accepts cash and credit/debit cards.

We are unable to accept checks.

Here are some tips to help you prepare:

•Make sure you know where our office is located. Click here for interactive directions.

•Write down what you would like to discuss with Dr. Friedberg, including any questions you have.
•Know the names of all of your current medications.
•Know what symptoms you are experiencing, when they started and how to clearly describe them.
•Bring information regarding your personal health history and the health history of your family.
•Write down all dates and names of procedures, tests and medications pertaining to your health history.
•Before you leave, make sure you fully understand Dr. Friedberg's recommendations and answers to your questions as well as the timing of any necessary tests or follow-up visits.
